Module introduction

This module will describe the content of the HHFA questionnaire and how it is organized. It will also outline the structure of the HHFA paper questionnaire.

By the end of this module, you will:

• describe how the questionnaire content is organized;
• list the categories of information collected in the service readiness sections;
• state the number of columns into which the questionnaire is divided;
• describe the kind of information you can find in each column.
